Terminology

  • Lentigines are due to localised proliferation of melanocytes, a type of skin pigmentation. The most common type, solar lentigines. Note: pigment is inside melanocyte NOT keratinocyte. 
  • The correct term for a single lesion is solar or actinic lentigo.

Melanin pigment production variation during different seasons

Freckle Lentigines
The melanocyte don’t produce much melanin when there is low amount of sun ray, especially during the winter months, or when people do not go outside due to cold. Lentigines tend to persist for long periods and don’t disappear in the winter (though they may fade).

 

Pigment seasonal behaviour

  • Lentigines tend to persist for long periods and don’t disappear in the winter (though they may fade).
  • Lentigines are common in those with fair skin but are also frequently seen in those who tan easily or have naturally dark skin.
  • solar lentigines, arise in middle age and also result from sun damage (age spots).

Freckle Lentigines
Melanin is dispersed inside keratinocytes cell, which is located at the most surface layer of epidermis. Melanin is contained inside deeper sited melanocyte cell, at deepest part of epidermis

 

 

 

Differential diagnosis

It is important to distinguish the harmless solar lentigo from

  • an early malignant melanoma,
  • subtypes lentigo malignaand lentiginous melanoma.

 

Lentigines and melanoma

  • If the freckle
    • has arisen recently,
    • is itchy
    • is made up of more than one colour or
    • has irregular borders or
    • changes of color or discharge
    • or if you have any doubts, see your dermatologist for advice.
  • It is sometimes difficult to tell a benign lentigo from a melanoma, particular if there are abnormal/atypical features. This is be it clinically or pathologically. This is called atypical melanocytic hyperplasia.
  • In such atypically or abnormal cases, the lesions should be completely removed by surgical excision and send for pathological examination to excludes any malignancy/cancer.
