The first generation of transplant was using the mini-graft or plug graft technique. Here, there is a lot of damage to the donor site, sacrificing precious hair follicle and creating large scar.
The recipient site also compromise in aesthetic value as the large chunk of hair follicle inserted are not natural looking, with patchy area of dense follicle and sparsity in between.
The tissue of donor site need to be sacrificed to allow new graft to be inserted affecting the scalp quality in term of elasticity. Furthermore, there is problem of getting the leveling right causing elevation and depression and hence unevenness on the recipient area.

Now, the latest generation of technique and technology is using the uni-follicular graft transplant, or follicular unit transplant, FUT in short. As the name suggest, it uses a single or 1-2 or maximum of 3-4 unit of follicles for transplant.
The customer’s hair is transplanted in group of 1-4 hairs called the follicular units. Each of the follicular unit usually contain other mini structure of skin tissue and organ such as the sebaceous gland, nerve, small muscle and even fine vellus hairs. The FUT gives a very natural look. This is because the planted hair is of the same natural density, and the distance between the hairs is quite constant.
The follicular unit is a distinct entity by itself anatomically and physiologically. This entity is preserved during the extraction from the donor site and will be planted into the recipient site as a whole unit as well without being damaged.
Hair normally grow in follicular unit of 1 to 4 hairs. Since the FUT is identical with the natural hair growth density and pattern, it therefore, give a very naturally looking hair after the hair restoration work. The big number of FUT harvested from the back allow the aesthetic surgeon enough number to work on when planting them into the front bald area.
The density of hair in a normal scalp is quite constant and usually measuring about 1 unit per mm2. With this knowledge, we will know how many we can harvest from the donor site, how many FUT we have to distribute evenly over the recipient area on the front and vertex area.
In the rare case of patient with thinner hair density, the aesthetic hair transplant surgeon can take precaution to preserve the hair of donor site for future use. The compromise is to preserve these donor site and increase the density in the bald area.
The main idea of a transplant is to have a naturally looking hair. In order to achieve this, we must preserve the scalp of both the donor as well as the recipient site, and the healing after the transplant must leave no scar and a normal healthy scalp.
The follicular unit is complex yet complete structure both anatomically and physiologically. This unit is usually surround by substantial amount of tissue, the scalp included. In order to create a perfect sung fit of donor inserted into recipient site, it is best to trim the extra scalp skin on the top but much of the rest intake. A small portion of the fat at the bottom must be preserved intake.
Latest generation extraction is also of uni-follicular as mentioned above. The extraction is so accurate that only 1 to 2 or maximum 2 to 3 hairs are extracted in one site using a special tiny ouch no bigger than 0.8 mm. The trauma on the donor site is virtually nonexistent. This allowed repeated extraction in a future session if the needs arises.
