Eye lift is a surgery also called lower blepharoplasty is done to correct the imperfection due to the loose skin, sagging tissues and protruding bag of fat due to weakness of the wall that prevented its protrusion.
Often, the patient themselves or their friend commonly tell them why they looks tired or sleepy due to the puffy lower eye lids. Very often patient going for eye bag removal also need upper blepharoplasty as most of them also have droopy upper eye lid.
Prolapsing fat: The main cause is due to the weakness of the wall that support the orbital fat where the eye sits. Many believe that the bag is due to water retention or allergic reaction but is not. This compartment of fat prolapses causing globule-like swelling referred to as bag. To correct this we need to remove the prolapsing fat or re-positioned and sculpted and strengthen the wall.
Loose skin: Another cause is due to the loose skin. In the early stage, the skin is not much loose and fat can be removed without removing the skin through trans-conjunctiva surgery. However, as the skin laxity increases, then the skin can be removed and tightened usually together with the orbicularis occuli muscle for a rejuvenated look. The orbicularis muscle function like a belt with elasticity that can contract after the surgery.
The lower eyelid support is slower getting weaker due to aging and gravitational pull. Adequately restoring the support by various techniques can help creating a more rejuvenated youthful look.
The eyeball is located in the hollow of the skull called as the eye socket. Within this socket is filled with the fat which works as a cushion for the eyeball so that any impact force will not cause the shock to the eyeball. There is a thin sheet of tissue which extends from the orbital margin peripherally and extend centrally towards the lower eyelid, is called as the orbital septum. The orbital fat is lying behind this septum.
Many customer complaint that their eye bag is worst during the morning. The reason is that after a night of sleeping flat on the bed, the orbital fat does retain more water. However, after a day of upright standing, the gravity pull and the increased circulation due to the day activity do help to remove the collected fluid.
As the person age, the eye lid skin becomes lax, orbital fat protrude giving an aged and tired look.
Dr David Ling will assess the skin laxity by skin pinch test. If the skin is not too lax, then an internal approach of fat removal called trans-conjunctiva blepharoplasty, without the need for external approach is done to remove the eye bag. If the skin is too much, then external approach, called the trans-cutaneous approach will be used to remove both the fat and the skin and tighten the orbicularis muscle.

The orbicularis oculi muscle is a very delicate muscle that support the lower eye lid much like the hammock. Its integrity should be preserved at all time. Excess removal, especially by new inexperienced doctor can cause a “rounding” or “scleral show”, or a pulling down of the eye lid called eversion or retraction.
